What is a dermal filler?

A dermal filler is composed of Hyaluronic Acid (HA) alone, also known as a skin booster,  or combined with a small amount of a synthetic substance is added called BDDE.

By injecting the BDDE dermal filler precisely into specific areas facial contouring and rhinoplasty is achievable.

Typically, you will see an improvement in the appearance immediately following treatment, and a more personlised appearance is achieved.

How is dermal filler prepared?

The dermal filler is pre-made, pre-packaged, and opened when ready to administer.

A sterile station is prepared for the administration of the dermal filler injections.

What type of dermal filler is used?

Dermal fillers are primarily made of Hyaluronic Acid (HA), a natural hydrating substance occurring in our bodies.

To create a dermal filler with density and longevity, the HA chains have to become connected, known as cross-linked.  To cross-link the HA strands, a small amount of a synthetic biodegradable substance is added, which is called BDDE.

The result is a temporary, soluble hydrogel, which is not transported away by the body for up to 1 year.

Some dermal fillers contain lidocaine. Please advise your practitioner if you have an allergy to, or have ever had a reaction to, any such anaesthetic.

What will the treatment involve?

Your aesthetic practitioner will complete a thorough consultation.

The area to be treated is cleansed and sterilised, followed by the application of the local anesthetic, then the dermal filler solution is injected into the area selected.

The treatment takes approximately 30 to 60 minutes, depending on the areas to be treated. Your practitioner will advise you during consultation.

The actual injectable procedure is very quick – a short, sharp pain during the injection may be felt.

Please advise your practitioner if you have an allergy to, or have ever had a reaction to, any such anaesthetic.

Are dermal injections safe?

Dermal fillers have been in aesthetic use for a few decades, and thousands of men and women have received dermal filler injections worldwide every year.

Dermal Filler injections are administered in such a way that it only affects the area of your skin injected and it does not reach any other part of the body.

Providing the dermal filler injections are administered by a qualified, competent practitioner, the risks are extremely minimal.

Dermal filler injections are administered by our senior aesthetician practitioner to ensure that quality and safety is paramount.

Does it hurt?

The actual procedure can be reasonably quick depending on the area treated.

A local anesthetic is applied to the treated area prior to the administration of the injections to reduce any risk of pain.

A mild discomfort can be experienced during the procedure from the small needle injections involved. However, each individual has a varied pain threshold in relation to needle insertion.

Care is taken to make the injection as comfortable as possible.

As with all injections there may be some mild temporary discomfort and redness in the direct injection site, which subsides quickly.

Cold applications can aid any discomfort post treatment.

How many treatments do I need?

One treatment is usually required to achieve the desired look. However, several treatments may be necessary in some cases. The dermal filler results lasts from 6 to 12 months.

Anti-wrinkle Injection may be recommended along with the Dermal Filler Injections as the filler can contour, reshape and create volume in the crevices. But the botulinum toxin in the Anti-wrinkle Injection can stop the repetitive action, which causes the fine lines and wrinkles.

How quickly will I see the results?

You will see results immediately after a single treatment.

Several treatments may be required to achieve desired results.

How long will the results last?

The results can last from 6 to 12 months.

Your practitioner will discuss the various options to suit your individual needs.

What are the possible risks & side effects?

Every procedure involves a certain amount of risk and side effects, which will resolve. However, you should be made aware of them.

The risk of side effects following treatment is reduced by consultation with an experienced practitioner.

Rare Risks

Skin infection (cellulitis), which presents as hot, red, shiny skin, and you may also feel unwell. In the case of suspected infection following dermal filler, you should contact your practitioner but also seek a medical assessment as soon as possible as you may need antibiotics.

Occasionally infection can form a swollen collection called an abscess. In this case you must again seek urgent medical attention.

If you suffer from cold sores (a herpes virus infection of the lip) these can sometimes be reactivated following a lip filler.

Dermal filler can lead to lumps or nodules and sometimes there may be unevenness or poor cosmetic result.

Filler can occasionally migrate over time and lead to a poor cosmetic appearance, you may require filler dissolving injections to remove migration if it becomes very noticeable or persistent.

Occasionally people can develop an unwanted inflammatory reaction to the dermal filler, and this can lead to a nodule formation called granuloma. This often presents as a delayed complication, several months after the treatment. Several treatment options are available for granuloma formation and you may require medical assessment.

Dermal filler can sometimes cause skin discolouration (Tyndall effect) if accidentally injected too superficially. If this occurs, then you will likely require dissolving injections to remove the filler causing the discolouration.

Rare complications include allergic reaction to the dermal filler or lidocaine anaesthetic, if this a component of the product. Very rarely clients may develop an anaphylactic reaction, which would require emergency medical assistance and transfer to hospital.

Rarely dermal filler can be injected into an artery, blocking the flow of blood to the tissue being treated. This is known as vascular occlusion, if untreated it is serious because it can lead to death of tissue (necrosis), which may require reconstructive surgery to correct. In the rare event of vascular occlusion, it would be treated with emergency injections of a filler dissolving drug called Hyaluronidase. This is an enzyme that removes the filler occluding the blood vessel and can usually fully reverse the vascular occlusion, preventing any long-term harmful effects. Your practitioner will be trained in how to use Hyaluronidase in an emergency. However, if treatment with Hyaluronidase is not successful then you would require urgent medical assessment by a doctor who is a specialist in aesthetic medicine.

The rarest of all complications of dermal filler is called filler blindness. This is where a small amount of filler can be injected into a blood vessel, break off and travel within the circulation (embolisation) and cause blockage of the artery or vein supplying the back of the eye. There have been less than 200 cases reported worldwide but you must be aware of this risk especially if you are receiving nasal or glabella fillers. Filler blindness is irreversible in most cases. In the event of this complication you should seek urgent assessment at the nearest eye hospital or medical eye unit.

Contraindications

Please discuss your health concerns with your practitioner prior to commencing treatment.

Allergy to wasps.
Infection at site of injection.
Pregnant, or breastfeeding.
Inflammation in the skin to be injected.
Blood clotting disorders.
Any active disease of the cardiovascular system.
Previous history of seizures.
Plan to undergo an operation soon.
recent facial operations or trauma.
History of keloid or hypertrophic scars.

Are there any other important considerations?

Limited or no clinical data exists regarding the efficacy and tolerance of this treatment in patients having a history of, or currently suffering from, auto-immune disease or auto-immune deficiency or being under immunosuppressive therapy. The practitioner shall therefore decide how to proceed on a case by case basis, according to the nature of the disease and its treatment and the need for monitoring post-treatment.

Your practitioner will discuss the need for a preliminary skin testing for hypersensitivity if necessary, or in the case of patients with severe or multiple allergies.

Patients on coagulation medication or other substances known to increase coagulation time must be aware of the potential increased risk of bleeding and haematoma during and following treatment.

Do not apply make-up or spray tan for at least 48 hours if possible.

Avoid swimming in public pools for 24 hours.

Avoid any exposure to a sauna, or heat treatments for 2 weeks.

Avoid hot tub or tanning for up to 24 hours.

Avoid a facial, rub or massage the treated areas for up to 24 hours.

Avoid consuming any alcohol for up to 24 hours

Avoid air travel for 2 weeks.

Wear sun protector SPF 30 minimum

Drink plenty of water.

The application of an SPF when exposing your skin to UV can prevent long term skin damage.
